Payment is expected at the time of service. We will work with many insurance whenever possible. We accept most forms of payment including major credit cards. Package plans are available.

Patients who purchase promotional offers will receive all the services described in the offer. Please note these offers are for new patients only and may not be accepted for existing patients.

Preparing For Your First Visit
Please print out the patient forms emailed to you and take your time filling them out. Be as descriptive and detailed as possible. If you are uncertain which forms apply to you, please call our office for clarification. For Davie call 954-473-8925 and for Fort Lauderdale call 754-200-1555.
Insurance Verfication
We accept many insurance plans in our practice. Each plan requires that we verify the benefits. Prior to your initial visit please fax the front and back of your insurance card to our office. Please include on your fax your full name, date of birth and phone number. For the Davie office please fax the information to 954-473-5993. For the Fort Lauderdale office please fax the information to 888-840-4325.
Treatment Expectations
If you have several health conditions, please list them in the order of importance. Please be realistic concerning treatment. It make take several treatment sessions to feel a change. Please create a treatment result diary to note daily changes in how you feel.
